Our Grapevine center is located on the 1st floor of the Clearview 1 Professional Building at Baylor Scott & White Medical Center - Grapevine near the intersection of Highway 114 and Ira E Woods Avenue.

We know that injuries are never scheduled, but relief can be. If you're injured, tired of having less mobility or experiencing aches and pains, let our licensed clinical experts help. 

As a multi-disciplinary therapy center, we offer physical, occupational/hand and speech therapy for individuals with musculoskeletal conditions, including strains, sprains and post-operative care. Our staff of licensed therapists specialize in orthopedic, neurological and cancer rehabilitation. We will work with you to craft an individual treatment plan to help you quickly return to daily activities, work duties and recreational pursuits.

Our Team

Matthew LeCluyse, P.T., MPT - Center Manager

Matthew is an experienced physical therapist and has been with Baylor Scott & White Rehabilitation for more than 20 years. He enjoys treating patients of all kinds. Matthew is experienced in orthopedic rehabilitation, sports rehabilitation, manual therapy, trigger-point dry needling, blood flow restriction training, ReVital cancer rehabilitation and vestibular rehabilitation. He has a passion for helping people get back to doing what they love.

Avani Johnson, OTD, OTR

Avani was born and raised in Duncanville, Texas, where her passion for rehabilitation emerged from assisting individuals with disabilities in fitness and recreation. She received her bachelor's in applied movement science, disabilities studies and strength and conditioning from the University of Texas at Austin. She obtained her degree in occupational therapy from the University of St. Augustine for Health Sciences. Avani enjoys treating patients of all ages, with the breadth of her experience being in chronic illness and disability.

Kara Drefs, MSPT

Kara has been a clinician for over 20 years. She graduated with her master's degree in physical therapy from the Mayo Clinic. She loves helping people achieve their own functional goals and walking with them on their rehabilitation journey. Physical therapy offers an active profession which combines problem solving, communication and creativity. Kara is an active volunteer for community sporting events and church choir.

Mariana Morales-Najera, P.T., DPT

Mariana is passionate about helping patients regain function, decrease dizziness and improve their quality of life through evidence-based practices. Originally from Chicago, Mariana moved down to Texas to escape the harsh winters and embrace a warmer climate. Her bilingual proficiency in English and Spanish allows her to connect with a diverse patient population. She is LSVT BIG certified and graduated from Ohio University with her doctorate in physical therapy. Mariana says, "I became a physical therapist to combine my love for science and helping others. Working in rehabilitation allows me to make a greater difference. I value meaningful conversations and getting to know my patients on a deeper level."

Paxton Burnside, OTR, OTD

Paxton brings high levels of laughter and fun into her treatment sessions. She empathizes with her patients, as she has been through occupational therapy herself. She has a passion for helping people navigate their new normal. Paxton received her doctorate in occupational therapy from Texas Tech University Health Science Center. She specializes in hand, wrist and thumb injuries, neurological and stroke rehabilitation as well as visual perception. Paxton became an occupational therapist to help people return to activities meaningful to them. It is extremely rewarding for her to see people progress and to have them tell her things they are now able to do. Her motto is: "My patient's goals are my goals!"

Rachel Bressler, M.S., CCC-SLP

Rachel has been a speech language pathologist for seven years. She loves the field of speech pathology because she gets to make connections with so many different people from different walks of life. Every day is different but finding out what is important to her patients and helping them reach their goals is fulfilling. Rachel is a student mentor and stroke group facilitator. She specializes in dysphagia, voice and upper airway disorders, neurological rehabilitation and cancer rehabilitation.

Shantha Nithiananda, P.T., DPT, NCS

Shantha is a Texas native who currently lives in Fort Worth. Although her initial interest in physical therapy came from her love of soccer (and two ACL replacements), she has a passion for helping neurologic and vestibular patients in her practice. She graduated from Baylor University with her bachelor's degree in 2016. In 2019, she completed her doctorate in physical therapy, followed by her neurologic physical therapy residency, both at Emory University. She is a board certified neurologic clinical specialist from the American Board of Physical Therapy Specialists and LSVT BIG certified physical therapist through LSVT Global.

Stephen Rodriguez, OTR

Stephen has worked as an occupational therapist for over 20 years. He is bilingual in English and Spanish and he enjoys treating all types of diagnoses. Stephen is experienced in splinting and positioning and has a passion for promoting wellness for all and educating on the benefits of occupational therapy. Outside of work he enjoys spending time with his family and dog.

Success Stories

Thelma Gumbert

In July 2023, Thelma and Paul Gumbert moved from the United Kingdom to the United States to be with their daughter. Before the move, Thelma wasn't in the best health. She had a stroke in June 2021 and broke her hip in March 2022. She began visiting our center in March 2024. Thelma was extremely motivated to complete her physical therapy exercises. When she first started coming to us, she was unable to climb in and out of the passenger seat of her automobile. Now, Thelma and Paul look forward to going out on dates without the assistance of their daughter!
